Reading it I remember a thought I had some time ago, probably on the occasion of reading Set This House in Order: theory of mind and dissociative identity disorder mark two ends of a continuum, and what writers do is somewhere in between. (Not all writers' subjective experience is as Bull describes, but I think that just means not all writers are at the same place on that continuum.)
There's probably a journal article or two in there if I could only think how to test it empirically.
